Description

Background: Major Depressive Disorder (MDD) is a life-altering and common disease that continues to rise in the United States. Current treatment guidelines include monotherapy or combination therapy of medication, psychotherapy, and electroconvulsive therapy (ECT). Although there are some studies that state Mindfulness-Based Cognitive Therapy (MBCT) is effective in treating MDD, it is not clear where it may fit in the therapeutic approach. It is important to examine MBCT as a treatment to MDD in order to determine if it may be used as a monotherapy in treating the prevalent disease. Objective: To determine whether MBCT can be used as a monotherapy in treating MDD by using an evidence-based medicine systematic literature review study design. Methods: This systematic review was conducted between October 2018 and June 2019. Three independent researchers reviewed search results against inclusion and exclusion criteria from four scientific databases. Following this a secondary review for highest level of evidence and application to the research question narrowed results to three articles. Qualitative analysis was performed in order to compile the results of each selected article and draw conclusions as to whether MBCT is sufficient as monotherapy to provide positive outcomes for adults with MDD. The three selected articles were meta-analyses of randomised control trials graded as 1A level of evidence. Results: Each article analysed found that MBCT yielded comparable results to standard treatment methods, which include the combination or monotherapy of antidepressants, psychotherapy, and electroconvulsive therapy. Conclusions: While it was not found to be superior to treatment as usual, there is evidence to support the role of MBCT in the treatment of MDD as a monotherapy when appropriate to the patient. Further research is necessary to determine the quality and duration of MBCT treatment so that recommendations can be made for an update to current guidelines in treating patients suffering from MDD
